1.1 Dr Raj Reddy respects your right to privacy and is committed to protecting the privacy of his patients and website visitors. We adhere to the Australian Privacy Principles contained in the Privacy Act 1988 (Cth). This policy sets out how we collect and treat your personal information.

1.2 “Personal information” is information we hold which is identifiable as being about you.

2. Collection of personal information

2.1. Dr Raj Reddy will, from time to time, receive and store personal information you enter onto our website, provide to us directly or give to us in other forms.

2.2. We need to collect your information to provide you with the best care including properly assessing, diagnosing and treating you.

2.6 We may have access to your medical records from time to time. We hold your information in hard copy and electronic forms.

2.7 In addition to collecting health information directly from you, we may also collect information about you from a third party such as other health service providers, a family member or legal guardian.

3. Use and disclosure of your personal information

3.1. Dr Raj Reddy may use and disclose personal information collected from you for the purpose of providing health services to you. The Australian health system often uses a multidisciplinary approach. Medical practitioners work together and share necessary information to deliver optimum health care. These medical practitioners include your general practitioner, other medical specialists, allied health professionals and medical investigation providers.

When an examination is performed under an insurance claim, your information is disclosed to either your employer or the insurance company concerned for the purpose of invoicing payment. Billing information may also be disclosed to Medicare Australia. In the circumstances of adverse incidents, your information may need to be disclosed to medical indemnity insurers.

3.2.  For other matters, we would not use and disclose your personal information without obtaining your consent.

3.3. We may from time to time need to disclose personal information to comply with a legal requirement, such as a law, regulation, court order, subpoena, warrant, in the course of a legal proceeding or in response to a law enforcement agency request.

3.4. Dr Raj Reddy may contact you by a variety of measures including, but not limited to telephone, email, sms or mail.

4. Data Quality and Security

5.1. You may request details of personal information that we hold about you in accordance with the provisions of the Privacy Act 1988 (Cth). We will grant access unless the Privacy Act 1988 (Cth) or other relevant laws allow us or require us to refuse this access.

5.2. We may charge a fee to recover reasonable costs that result from supplying information to you.

6. Use of Identifiers

6.1 We will only use your Medicare number for the purpose of billing for medical services provided.

6.2. We would not use or disclose any other government identifying number which you may have given us for any other purposes.

7. Anonymity

7.1. Where lawful and practicable, you can use health services without identifying yourself.

8. Transborder Data Flows

8.1. Your information will not be transferred outside Australia unless the intended receiving country has a similar privacy regime or only if you provide your consent.

9. Complaints

9.1. If you have concerns about the way we have handled your personal information, please contact our administration staff.

9.2. We prefer that your complaint is in writing. It is our intention to resolve any complaint fairly and as quickly as possible. We take complaints very seriously and will respond shortly after receiving written notice of your complaint.
